Trinity Health Academy considers applicants for admission based on the following criteria:
1. High School diploma or GED – Prospective students must sign an attestation stating that they have either a high school diploma or GED (high school equivalency).
2. Completion of admissions application and submission of all required documents.
3. Applicants must achieve a passing score on basic skill exam. Test scores are valid for one year from test date. A minimum passing score of 70 is required for admission to the program.
4. Be at least eighteen (18) years of age on or before the first day of class.
5. Two (2) forms of identification: one must be photo identification: (driver’s license, passport, employee ID) and required social security card.
The entrance exam re-test requirements are as follows:
- After a first attempt, an applicant may re-test after 2 weeks.
- You can only test 3 times per year. Each additional test must be at least 4 weeks apart.
Additional Admission Requirements
In addition to the general admission requirements stated above, the following program entrance requirements must also be met:
- Must provide health form, proof of immunizations, Hepatitis B vaccinations or waiver, covid-19 vaccination and booster, and 2-step PPD, within the first 14 days of class.
- Applicants must successfully pass a criminal background check.
Upon acceptance a seat deposit of $170.00 must be made within 7 days. The balance will be due one week prior to start of class.
